The Institutional Review Board (IRB) has revised the application form and requirements for annual study continuation submissions. The new application form can be found on the IRB website at http://www.wichita.edu/j/?2677. For studies requiring written informed consent, the IRB now requires copies of the last three consent forms signed by enrolled subjects be submitted along with the continuation application.

This is to verify that the investigator is using the most recently approved version of the consent form and is appropriately obtaining consent from participants. Once this is confirmed, the IRB staff will shred the signed forms so as to protect the privacy of the participants.
