The deadline has been extended to Saturday, Nov. 15, for employees who have not yet qualified for the incentive discount on their 2015 health insurance premium. Those who complete the required health assessment questionnaire (worth 10 credits) and earn an additional 20 credits (for a total of 30 credits) by Nov. 15, will qualify for a $480 premium incentive discount for plan year 2015. Confirm your discount status by logging in to your account at http://www.KansasHealthQuest.com and clicking on the "My Printable Credits Summary" link from the left menu. Go to http://www.kdheks.gov/hcf/healthquest/rewards.html for full details on the rewards program. The next HealthQuest program year will begin on Nov. 16, 2014.
