Because of the Affordable Care Act, a new form will be required for all employees when filing your 2015 taxes -- the IRS 1095-C form. The State of Kansas mailed forms to your homes on Tuesday, Jan. 26. Put this form with your other tax-related documents.

Similar to a W-2 or 1099 form, you will need to have it if you prepare your own taxes, or provide it to your tax professional who helps prepare and file your taxes. You may also log in to https://sehp.member.hrissuite.com/ and print one off.

Login credentials match your Open Enrollment login ID and password. If you have forgotten your login information, you can re-register by clicking on the "Register Now" button. Once logged in, the form is located on the Forms tab on the Account Overview screen, under "Tax Forms."
