The State Employee Health Plan began mailing your required Employer-Provided Health Insurance Offer and Coverage 1095-C forms as of 1/10/17. Forms were mailed to the preferred address listed on the Membership Administrative Portal (MAP). You can also access this form by logging into the MAP portal at https://sehp.member.hrissuite.com/.
