Please don't forget that the Health Assessment is required for employees and enrolled spouses to earn the 2018 Premium Discount through the HealthQuest Rewards Program. By participating in the required Health Assessment, you will receive 10 credits and Plan C members will receive an HRA / HSA contribution of $100.

Plan A and Plan C employee and employee / children coverage will earn a $480 reduction for 2018 and employee / spouse and employee / family -- both the employee and spouse can earn a $240 reduction ($480 total) for 2018. Plan C members will also earn HRA / HSA dollars per activity (up to $500 per employee and spouse) to be deposited in 2017. Please continue to track your completed wellness activities in your HealthQuest wellness portal through Dec. 31, 2017, to receive your full premium reduction (Nov. 9, 2017 for Plan C HRA / HSA contributions).
