The W4 form for 2018 has been added to the Student, Grad, Lecturer and Unclassified Professional Temp hiring packets. Also, WSU Policies and Procedures 9.10/Intellectual Property and Institutional Procedures, which defines disclosure, ownership and sharing of profits, requires all new hires complete the Acknowledgment - Patent Ownership form and the New Employee - Intellectual Property Questionnaire. Failure of new hires to complete these forms will place WSU out of compliance with internal policies.
