According to the Office of Human Resources, all HR Payroll/Employment forms and instructions have been updated. The revised forms have discontinued any reference to Social Security numbers, added fields to indicate exempt/nonexempt status, and the appointment forms have a box to indicate when the background check has been completed. A new form has been added, the OHR 420/Research Change of Effort and Funding, which will be used to show any change in effort and funding of a position or employee that is partially or totally grant funded, contract funded or research.

Forms with a revision date older than Sept. 4, 2009, should no longer be used. If you have downloaded the old forms to your computer, please delete them and use the revised forms.
