ADA Mobility Accommodation Request Form

If you require mobility assistance to and from classes due to a mobility limitation, please complete the ADA Mobility Accommodation Request Form. Be sure to provide all required information and fill the form out as accurately as possible. Additionally, please attach a copy of your current class schedule, or a list of the classes you will need assistance getting to and from. We would like to pre-schedule rides to classes or meetings,
Mobility accommodations are provided through the use of golf carts operated by Transportation Services staff.
