Guest Artist Recital, Master Class, Faculty Artist Recital, and Studio Recitals Request Form
Complete this request and the Guest Artist Contract Information Form four to six weeks before the event.
This form must be completed and approved before requesting rooms and/or Performance Facilities services.
This form must be completed by School of Music Faculty only.
If approved, the faculty member and Performance Facilities will receive confirmation of the request and may make arrangements for rooms and/or Performance Facilities services. This must be done at least four weeks in advance. It is also the responsibility of the faculty member to arrange to have the piano tuned, at least three weeks in advance.
Any Guest Artist, outside of the College of Fine Arts, may be required to sign a Guest Artist Authorization Form provided by Performance Facilities if the performance will be audio recorded or live streamed.
If this request is declined by the School of Music, the performers are responsible for payment to Performance Facilities.
