Guest Artist Information Form

This form requests the creation of a contract, it is not a contract.

This request may also require the completion of an Adding Performances Form, if applicable.  These must be submitted at least four weeks before the event.

A W9 from the artist is required.

After the contract has been created, signed by artist/manger and returned to WSU, paymnet will be requested.

This form must be completed by a School of Music faculty member.
