There are three different Service Agreements that correspond to your organization type.
General Organization Testing Services Agreement
Best for institutions such as, but not limited to, most businesses or organizations,
including community organizations and nonprofits. Not for medical providers/institutions
or governmental departments. (Updated March 2022)

Healthcare Provider Test Services Agreement
Best for institutions such as, but not limited to, hospitals, nursing homes, emergency
care clinics, and specialty surgery centers etc. (Updated March 2022)
Bulk Government Testing Services Agreement
Best for institutions such as, but not limited to, county agencies, city agencies,
fire, police, and EMS departments, public universities, and Kansas Unified School
Districts, etc. (Updated March 2022)

A standing order for testing is provided by Dr. Mark Leiker, WSU MDL physician, for all testing done through WSU MDL.
If you are a healthcare provider who would like to use your own physician's order for testing, please email the information to mdl@wichita.edu.
Once all registration information is received by the MDL, our account management team will create your organization in our system.
We will:
--Create your Organization and Registered User accounts
--Create a physician in our system and apply testing order (if using a physician
other than WSU MDL physician, Dr. Mark Leiker).
Once your account has been created, you can View Patient Results/Requsition Samples.
MDL will contact you with a client code required to order specimen collection kits.
Collection Kits are available to organizations who have an executed Testing Service Agreement with the Wichita State University (WSU) Molecular Diagnostics Laboratory (MDL) and are a registered testing partner.
*When the Federal CARES funding expires and/or State of Kansas SPARK funds are depleted organizations will be responsible for costs. WSU MDL will notify ALL testing partners when any changes are made to funding.
All Registered Users are required to complete Onboard Training in order to be trained on our software, as well as best practices for safely collecting specimen samples. Instructions to join an onboarding training will be emailed to your organization.
