On-campus Housing Residents: Uploading Meningitis Vaccine Required Documentation

  1. Sign into the Student Health Portal using your myWSU ID and password at www.wichita.edu/shs or the myStudentHealth portal button above.
  2. Verify your account with your birthday
  3. Once on your portal page, immunization requirement for housing will be uploaded through the "Requirements and Forms" tab. 
  4. Requirements & Forms will only have items specific to you. Here is where you will find the immunization document requirements for the Housing and Residence Life Meningitis Vaccination. You will be entering dates of your required immunizations and also uploading immunization records. 
  5. Under "Items required for Clearance", Immunization Documents and Meningococcal ACWY are listed. Click on the Meningococcal ACWY "Update" button to enter and upload vaccination information. 
  6. In blue, you will read exactly what is acceptable to meet the Meningococcal vaccination documentation requirement. Click on "Add Immunization". Enter the date of your most recent meningitis vaccination administered to you AFTER the age of 16 and pick the name of the vaccination you received. Click "Save"
  7. You will now upload your immunization document as instructed. Please read what is required and what are acceptable formats. Click "Upload".
  8. You will receive a preview look at the document, make sure it is legible and includes your name and birthdate. Click "Looks Good". Once you have the date entered and immunization record uploaded, click "Done"
  9. Under Items required for Clearance, the status will still say "Not Compliant" in red, but now details will say "Awaiting Review". If details say "No Data", your documentation may not have been accepted or the upload did not work, try again. Our SHS staff will review your uploaded information in 48-72 hours or sometimes longer during busy times of year. 
  10. If you have a religious or medical exemption, please email student.health@wichita.edu to request a Meningitis Vaccination Waiver. You will also upload a completed and signed waiver into the portal. Follow "Exemption" instructions.
  11. Once your items are reviewed, if they have been acknowledged and accepted, your status will change to "Compliant" in green. The details will change to "Satisfied". 
  12. If your items have not been changed to compliant and still say "No Data", please review your "Messages" tab in the portal. A SHS staff member who has reviewed your documents will message you with details of why your document was not approved or why you are stil non-compliant.  
College of Health Profession Programs: Uploading Health Requirements for Clinicals

  1. Sign into the Student Health Portal using your myWSU ID and password at www.wichita.edu/shs or the myStudentHealth portal button above.
  2. Verify your account with your birthday
  3. Once on your portal page, immunization requirement for housing will be uploaded through the "Requirements and Forms" tab. 
  4. Requirements & Forms will only have items specific to you. Here is where you will find the listed requirements for your College of Health Professions program. You will be entering dates of your required immunizations, physical exam and tuberculosis tests and also uploading the formal documentation you received from your healthcare provider.
  5. Under "Items required for Clearance", Immunization Documents and all items required are listed. Click on one of the item buttons to enter and upload vaccination information. Our example is for the Hepatitis B vaccination requirement. 
  6. In blue, you will read exactly what is acceptable to meet the Hepatitis B vaccination documentation requirement. Each required item also includes this information and includes how many vaccine dates or lab result information is needed. Click on "Add Immunization". Enter date of your Hepatitis B vaccinations and pick the name of the vaccination you received. Continue to make sure all 3 required Hepatitis B dates are listed. Click "Save"
  7. If you had to have a blood test to prove immunity for any required line items,  you will enter the dates of the lab results. 
  8. You will now upload your immunization document and lab results as instructed. Please read what is required and what are acceptable formats. Click "Upload".
  9. You will receive a preview look at the document, make sure it is legible and includes your name and birthdate. Click "Looks Good". Once you have the date entered and immunization record uploaded, click "Done".
  10. Continue to enter the dates of each required vaccination, blood titer test or tuberculosis test and upload a document when requested. Please double check to make sure that if 2 or 3 vaccine dates are required, that you have entered all dates (Ex: Hepatitis B requires 3 vaccine dates). If you need a medical exemption for any item, please contact your health profession program coordinator. 
  11. For a copy of the physical exam document to give to your healthcare provider, click on the physical exam in your required items list to "Download" a copy of the form.
  12. Once your physical exam has been completed and signed by your healthcare provider, you will "Upload" the completed form. 
  13. Under Items required for Clearance, the status will still say "Not Compliant" in red, but now details will say "Awaiting Review". If details say "No Data", your documentation may not have been accepted or the upload did not work, try again. Our SHS staff will review your uploaded information in 48-72 hours or sometimes longer during busy times of year. 
  14. Once your items are reviewed, if they have been acknowledged and accepted, your status will change to "Compliant" in green. The details will change to "Satisfied". 
  15. If your items have not been changed to compliant and still say "No Data", please review your "Messages" tab in the portal. A SHS staff member who has reviewed your documents will message you with details of why your document was not approved or why you are stil non-compliant.  
  16. Download one copy each of your "Requirements and Forms" page AND your "Immunizations" page, both pages can also be found on left side of portal, and submit your program. 
Submitting Insurance Cards
  1. Sign into the Student Health Portal using your myWSU ID and password at www.wichita.edu/shs or the myStudentHealth portal button above.
  2. Verify your account with your birthday
  3. Select "Insurance Card" in the left hand menu
  4. Select "Add New Card"
  5. Fill out information as best you can. **Please include in "Member Name" the policy holder and what relationship you have with them. For example Wu Shock's primary policy holder is his mom, Wucy Shock. He would enter Wucy Shock, mother, in the member name. 
  6. Select "Upload Front Image"
  7. Select file
  8. Select "Open"
  9. View document, if it is correct select "Looks Good" if not "Cancel Upload" and repeat steps 6 through 9
  10. Select "Upload Back Image"
  11. Repeat steps 7 through 9
  12. Select "Save"
Submitting all other health records
  1. Sign into the Student Health Portal using your myWSU ID and password at www.wichita.edu/shs or the myStudentHealth portal button above.
  2. Verify your account with your birthday
  3. Select "Messages" from the left hand menu
  4. Select "New Message"
  5. Select "NEW! I want to submit documentation or records" ** if you have a provider you have been working you can choose "I want to communicate with a Medical Provider I have seen recently"
  6. Select "Continue"
  7. Fill in "Subject" and compose a message
  8. Select "Add attachment"
  9. Select file
  10. Select "Open"
  11. Select "Send"
