A MAP form is used when you have multiple forms of the same exam (up to 3 forms; A, B, & C). In other words, students can receive up to three different question arrangements for the same test. Students need to indicate in the Form Box of their bubble sheet which form of the exam they used. The instructor will fill out a bubble sheet for the key for form A. Then fill out the MAP form for B, etc.
